I found it to be a great advice to start on Hardcore right away when I bought the game. Not only it was somewhat challenging, but also unlocked nightmare mode and item shop which were a lot of fun. The one thing you've got to swallow is a general comfort with and advocacy for violence in many forms. Most of it's fantasy, a lot of it is not. Kinda like hip-hop in the 80s/90s.
